The Pre-Screening Form is the record you complete for every person tested, before the test starts and once it is finished. Over time the pad becomes your permanent record book for your workplace drug and alcohol testing program. Each form is a single A5 sheet with simple instructions printed on it, so testers capture the same information every time.
